- Original Uncancelled Grade 10 Report Card (Form 138)
- Original Copy of Education Service Contracting (ESC) Certificate or Original Copy of Qualified Voucher
Recipient Certificate (if applicable) - Photocopy of Certificate of Completion or Diploma

TRANSFER STUDENT / SECOND COURSE
- Original Certificate of Good Moral Character from school origin
