Original Uncancelled Grade 12 Report Card (Form 138)
Photocopy of Certificate of Completion or Diploma
4 pcs of identical 2×2 photos with name plate and white background
Data Privacy Policy*
Undertaking Contract*
Promissory Note* – In case of lacking requirements, student may enroll with only school ID or original
PSA birth certificate. We’ll provide a signed promissory note for any missing documents (form included).
